The Colorado Public Health Medical Home Advisory Council has been in existence since 2007 and is a community collaboration of more than 40 cross-sector partners with a mission to:

	Inform, educate and empower partners about matters of interest to the medical home community
	Mobilize community partnerships to identify and solve problems, and barriers related to systems building a medical home team approach
	Link partners to people, information and resources that support our mutual objective to ensure all Colorado children have access to a medical home team approach.
